To submit a prayer request, simply fill out the form. Please include the prayer recipient’s first and last name, the complete mailing address of where you wish to have the prayer shawl/lap blanket and/or cards sent (please include unit number, if applicable), a contact email address, and the name and relationship of the person submitting the request. We ask for the type of cancer they are facing and a brief bio of the prayer recipient, to aide those who are praying for you or your loved one. To ensure a prompt response, please fill out the request completely.
Prayer Cards or Online Prayer Companion
Opt for your loved one to receive messages of prayer, love and encouragement by registering them for either the Prayer Cards or for an Online Prayer Companion.
Prayer Cards:
A Bio card will be crafted with their first name and the initial of their last name with the type of cancer they are facing. Inside the card speaks as to how we can pray for them, attached are notecards that visitors can take and share prayers and kind words with the recipient. These cards are placed on the prayer wall in the lobby of the Shrine. Volunteers mail out these notes of prayer, love and encouragement over a period of 28 days.
Online Prayer Companion:
To register for an Online Prayer Companion you must submit an email address which the prayer recipient has given permission to share with us. An online prayer companion commits to praying specifically for the needs of the person facing cancer over a period of 28 days. Reaching out, via email, at least once a week with messages of love and encouragement.

Prayer Shawls & Packets
Prayer shawls/lap blankets are crafted in prayer. Volunteers knit, crochet or quilt these gifts. Father or Deacon then pray over them and bless them with the sprinkling of Holy Water. Our Volunteers then take them and pray through the intercession of St. Peregrine for the recipients of these gifts and are touched individually to the First Class Relic of St. Peregrine. They are then ready to be sent out as Third Class Relics of St. Peregrine!
You may also request a prayer packet. They contain a Blessed Rosary & Brown Scapular. Also included is information on how to pray the Rosary & Divine Mercy Chaplet, as well as what the Brown Scapular is.
